I google it like for an hour and can't find Canadian (Ontario) version, so I will ask it.

do you have link to salary (money) calculator to calculate how much contractor should ask to get amount of money same as on permanent position (including benefits).
would be nice to have it showing also taxes per income (Ontario 2012).

такого калькулятора нет. Как вы бенефиты permanent высчитаете в конкретной цифре? Они везде разные. Что лучше - машина или дентал?
контракторы тоже разные бывают, кто то может списывать больше, кто то меньше
каждый раз условия нужно смотреть и решать

грубо на 2 умножай, т.е. $50 per hour примерно где-то 100K annualy при долгосрочном контракте

I never been on contract. Talk to recruiter today. She said that first contract is the cheapest one.
Salary for second contract will go up.

Is it true ? Why is that ?

не знаю что именно рекрутер имел в виду, возможно что с опытом и зарплата растет
при желании рейт можно достаточно высокий получить и на "первом" контракте, все ж относительно
