We just recently had a huge increase (30%) in our premium so I would like to know is it reasonable to pay $130 per month (family rate) to get this:Gatchinskiy писал(а):... а зачем, в чем поинт? ... какие-то вопросы или просто? ... все зависит от ковераджа ...

Chambers of Commerce Group Insurance
LIFE INSURANCE
Your Group Plan provides a $25,000 Life Insurance benefit, payable to the beneficiary of your choice. This is 24 hour coverage.
Monthly Premium: $ 5.87
ACCIDENTAL DEATH & DISMEMBERMENT
If you die as a result of an accident, the $25,000 benefit doubles. It also pays a benefit for accidental loss of limbs, sight and use of limbs. This is 24 hour coverage.
Monthly Premium: $ .75
EXTENDED HEALTH
The plan covers eligible expenses not paid by the provincial plan up to limits specified in the contract. Eligible expenses include paramedical, private duty nursing, semi-private hospital and ambulance services, plus out-of-province and -country insurance. Drugs are covered at 80% if they require a prescription. You have family coverage with a total deductible of $50 per year.
Monthly Premium: $ 55.89
DENTAL
The plan covers 80% of Basic, Endodontic and Periodontal services like fillings, check-ups, x-rays, basic restoration and minor surgery. The maximum benefit is $1,500 per person per year. You have family coverage with a total deductible of $50 per calendar year.
Monthly Premium: $ 62.22
Total is 124.73. Half is paid by employer.

The amount that I pay every month ($130) is my half, we also have 50/50 sharing-cost with the employer.

Мой дантист говорит, что если все маленькие дырочки аккуратненько и своевременно филить, то до страшилок, типа рут канала дело никогда в жизни не дойдет. Считаю, он в этом прав -- или он чего-то не знает, и зуб может мгновенно сгнить до корня?

23.04 - Extended Health Care Plan
(a) The Company shall pay the monthly premium for regular employees entitled to coverage under a mutually acceptable extended health care plan.
(b) The maximum life time benefit for each covered family member for medical benefits is $40,000.00
(c) The Company will amend its Extended Health Benefit Plan provisions to provide the following:
1. Hearing aids - $700.00 per ear payable once every 48 months;
2. Corrective lenses - $125.00 payable once every 24 months for adults and once every 12 months for children.
3. Diabetes - the cost of needles or, where prescribed by the employee's physician, needleless insulin injectors. Where needleless insulin injectors are purchased, the maximum payable will be $500.00 once every 60 months and cost of needles will not be claimable during that 60 month period.
4. Fees of a registered psychologist - up to a maximum of $500.00 claimable per family per year.
5. Hairpieces for chemotherapy patients - not to exceed $200.00 claimable every 24 months.
6. The amount reimbursed for any drug prescription shall be restricted to payment for the low cost alternative and/or reference based priced drugs, as identified by the Pharmacare Program.
7. Acupuncture coverage to a maximum claimable of $100.00 per employee per year and $250.00 per family per year.
(d) “Payable” means that the capped amount is reimbursable in full and the Plan deductible amounts and co-insurance percentages do not apply.
(e) Coverage shall cease on the day an employee’s employment terminates, excluding retirement on the Public Service Pension Plan or Long Term Disability, provided the premiums are paid by the Pension Plan or the LTD carrier, as applicable.
23.05 - Dental Plan
(a) The Company shall pay the monthly premium for employees entitled to coverage under a mutually acceptable plan which provides:
1. Plan A, 100% coverage;
2. Plan B, 65% coverage;
3. Plan C, 50% coverage.
(b) Orthodontic services are subject to a lifetime maximum payment of $5,000 per patient.
(c) The cleaning of teeth (prophylaxis and scaling) shall be every nine months except dependent children (up to age 19) and those gum diseases or other dental problems as approved by the plan.
(d) Coverage shall cease on the day an employee’s employment terminates, excluding retirement on the Public Service Pension Plan or Long Term Disability, provided the premiums are paid by the Pension Plan or the LTD carrier as applicable.
23.06 - Group Life Insurance Plan
(a) The Company shall provide a mutually acceptable group life plan with benefits equivalent to three times an employee's basic annual salary, with $65,000.00 minimum. The Company shall pay 100 per cent of the premium on the $65,000.00 base and the employee shall pay the premium for any insurance over $65,000.00.
(b) As a condition of employment, all eligible employees shall enrol in the group life plan and shall complete the appropriate payroll deduction authorization forms.
(c) Coverage shall cease on the day an employee’s employment terminates, excluding retirement on the Public Service Pension Plan or Long Term Disability, provided the premiums are paid by the Pension Plan or the LTD carrier.
(d) The group life plan shall include the following provisions for the accidental loss of:
1. both hands or feet the principal sum
2. sight of both eyes the principal sum
3. one hand and one foot the principal sum
4. one hand or one foot and sight of one eye the principal sum
5. one hand or one foot one-half the principal sum
6. sight of one eye one-half the principal sum
7. life the principal sum

Yes. You are right; I am working in the small company. We are happy to have 50/50 sharing cost. I should not complain about the coverage, it’s pretty good, but I think it’s pretty expensive too.

В прошлом году пришлось только за зубы за троих заплатить около 10к
